- Gender
- Are you a citizen of India? (Yes/No)
- Mobile Number (WhatsApp preferred)
- Email (if available)
- Current Address (door/street, village/town, district, state, pincode)
- Permanent Address (if different)
B. Education Details
- Present Class / Course / Year / Semester
- Institution Name and Address
- Institution Type (Government / Government-aided / Private)
- Board / University
- Last Year’s Marks (%) or CGPA
- Attendance % (approximate)
- Institution Contact (office phone/email)
C. Family & Income
- Father/Guardian Name, Occupation, and Mobile Number
- Mother’s Name and Occupation (if applicable)
- Annual Family Income (₹)
- Family Situation (Single parent / Orphan / Differently-abled / Serious illness / Other hardships)
D. Support Required
- What type of support you need (tuition fees / exam fees / books & uniform / hostel / transport / other)
- Approximate Amount Required (₹)
- A short letter (one page) explaining:
- Why this support is important for you
- How it will help your studies and future
2. Signature
- The student must sign at the end of the application.
- If possible, parent/guardian should also sign.
3. Scan and Send
- Take a clear scan of the entire application (all pages).
- Send it as PDF
4. Next Step
- After receiving your application, the Trust will review it.
- Verification will be done before sanctioning any support.
- Shortlisted students will be contacted directly.
